1) Identification & Emergency Contact Form (Mandatory) Due to increased security measures, the U.S. Coast Guard requires that each person (adults and children) traveling aboard our vessels complete an Identification & Emergency Contact Form.
Please complete the form as soon as possible (but no later than 60 days prior to your departure). If the information is not received, the Coast Guard reserves the right to deny passengers to board the vessel. Completing this form takes only a few minutes and can be done by clicking on the link below. That's it.

2) Travel Information Form (Mandatory) InnerSea Discoveries / American Safari Cruises can not book your flights to and from your home city. You or your travel agent are responsible for making those flight arrangements.
Once you have confirmed travel plans (NOT booked by InnerSea Discoveries / American Safari Cruises), including flights, rail, driving to the port, staying with friends, hotel, etc., please submit a Travel Information Form.
Don't "miss the boat!" Make sure your flight times meet our recommended arrival & departure guidelines.

3) Special Dietary & Medical Needs Form Submitting this form helps us identify any special accommodations needed as it relates to your diet, disability, or medical condition. Our primary concern is the health, safety, and well-being of our guests and crew.
We will do the very best to address your dietary needs and concerns, and recommend you speak with the Chef once on board. Keep in mind that due to the areas we cruise in, there may be limited special product availability. Please ask to speak with the Chef once on board. We can accommodate diabetic, low cholesterol, low fat, sodium-free, lactose-free, gluten-free, vegetarian, vegan, and food allergies.
We do not discriminate on the basis of disability. Guests with serious medical conditions or women in the third trimester of pregnancy should consult their physician to determine if travel is appropriate. We are not able to provide guests with personal devices, such as wheelchairs, or services of a personal nature, such as assistance in eating, toileting, or dressing. Guests are responsible for arranging such services and are encouraged to travel with a companion for such purposes.
